People with alcohol addiction have long complained that even the sight of a bar sign or a photo of a drink makes them reach for a glass. Science has now confirmed that this is not simply a lack of willpower — it is a real, measurable response in the brain.

According to a study published in issue 310 of the international journal Physiology & Behavior, a research team led by Dahyeon Kang and Silvia Murgia at the University of Illinois Urbana-Champaign found that patients with moderate-to-severe alcohol use disorder showed markedly different brain responses when viewing images of alcoholic drinks compared with images of non-alcoholic beverages.

A brain that responds only to alcohol

The research team showed 47 heavy drinkers between the ages of 21 and 32 alternating images of alcoholic drinks — bottles, wine glasses and cocktails — and non-alcoholic beverages such as juice, water and coffee, while measuring the brain's electrical signals. When an unexpected stimulus appears, the brain emits a specific electrical signal within 0.3 seconds; the stronger that signal, the more attention the brain is devoting to the stimulus.

The results differed sharply by group. Among the 20 patients with moderate-to-severe alcohol use disorder, brain signals were statistically significantly stronger when viewing alcohol images than when viewing juice or water. Among the 27 participants with no or mild symptoms, there was almost no difference between the two types of images.

The more striking detail lies in what drove that gap. In the severe group, the brain did not react especially more strongly to alcohol images. Rather, brain responses to ordinary beverages such as juice and water had diminished.

Compared with the mild-symptom group, the brains of severe patients had grown increasingly numb to non-alcohol stimuli, leaving alcohol-related cues as one of the few stimuli that could still move them at all.

How much they drank — they didn't know themselves

The research team also tracked how these brain responses connected to actual drinking behavior.

Participants wore a SCRAM ankle-mounted transdermal sensor for two weeks to continuously measure blood alcohol concentration. They were also asked to self-report their drinking — whether they had consumed alcohol and how much — via a smartphone survey eight times a day.

It was not the self-reported figures that predicted the strength of the brain response. Only the sensor readings showed a statistically meaningful relationship.

Among the severe-patient group, the more days of heavy drinking and the higher the peak daily blood alcohol level recorded by the sensor, the stronger the brain response to alcohol images. No such association appeared when drinking was measured by the participants' own self-reports.

The reason is straightforward. People who are intoxicated cannot accurately remember how much they have drunk. Memory becomes distorted, and social pressure leads them to underreport. The transdermal sensor captured the concentration of alcohol excreted through sweat without any such distortion.

Until now, the severity of alcohol use disorder has been assessed mainly through patient self-reports and clinical interviews. This study raises the possibility that measuring the brain's electrical signals could serve as an objective tool to supplement those methods.

The research team was candid about the study's limitations. The sample of 47 participants is small, and the observation period was limited to two weeks. With an average participant age of 22.6, the findings may not translate directly to long-term patients who have been drinking for decades.

The team said further research at a larger scale and with longer follow-up is needed to determine whether these brain signals can predict treatment response.
