Data from the US ClinicalTrials.gov registry show 588,044 trials registered worldwide, with the United States and China leading; drug and biopharmaceutical trials account for nearly half of all interventional research; South Korea has conducted 17,364 trials, with Phase 3 trials dominating

The number of clinical trials registered worldwide has surpassed 580,000, with South Korea crossing the 17,000-trial mark and cementing its role as a key player in global drug development. Domestic trials are heavily concentrated in late-stage research — particularly Phase 3 trials, which sit just before commercialization and carry the highest probability of success.

The Korea Biotechnology Industry Organization's Bio-Economy Research Center analyzed data from ClinicalTrials.gov, the world's largest clinical trial registry, operated by the US National Institutes of Health. The analysis found that 588,044 clinical trials had been registered globally as of Thursday.

Global clinical research has grown sharply since the site launched in 2000 with just 2,119 registered trials, surpassing 100,000 for the first time in 2010 with 100,202 entries. The registry stood at 520,862 at the start of last year, added 42,966 new trials over the course of the year and reached 563,828 by year-end, reflecting continued rapid expansion.

By country, the United States led by a wide margin with 192,401 trials, followed by China with 51,329. The two nations dominate the global clinical trial landscape.

By research type, interventional studies — those aimed at treating disease — accounted for the largest share at 448,711 trials, or 76 percent of the total. Observational studies, which track the progression of disease, followed with 137,305 trials, while expanded access studies for compassionate use of pre-approval drugs numbered 1,051.

Within interventional studies, drug and biopharmaceutical trials totaled 217,750, representing 48.5 percent — nearly half — of the category. Medical device trials numbered 62,083.

South Korea conducted a total of 17,364 clinical trials. The country's clinical trial ecosystem is characterized by infrastructure concentrated in late-stage and interventional research — the final gateway before regulatory approval and the stage at which therapeutic efficacy is ultimately validated.

Trial-phase data show 2,530 Phase 1 trials and 3,264 Phase 2 trials, while Phase 3 trials — the stage immediately preceding marketing approval — recorded the highest count at 3,973.

Phase 4 trials, which track post-market safety, numbered 1,553, with 3,856 classified under other categories. By research type, interventional studies were overwhelmingly dominant at 14,309, followed by observational studies at 3,034 and expanded access studies at 21.

By sponsor, industry-led trials — conducted by pharmaceutical and biotech companies — numbered 9,533, edging out the 8,997 trials led by universities and research institutions.

"The real-time expansion of global clinical trial data is an intuitive indicator of the intensifying worldwide race in drug development," an industry official said. "The concentration of South Korea's clinical trials in late-stage research such as Phase 3, and in company-led interventional studies, is evidence that the domestic pharmaceutical and biotech industry has matured considerably in its capacity to deliver commercial results."
